The RN Case Manager for the MOUD program provides advanced professional nursing care to patients receiving services for opioid use disorder, including medication (e.g., Suboxone, Sublocade, Brixadi, and Vivitrol) through the assessment, coordination, and implementation of the patient’s treatment needs. The RN Case Manager provides direct care to patients and their families, in partnership with the MOUD Coordinator, behavioral health counselor, and medical staff, as well as provides education and community outreach.
Qualifications/Skills
- Graduate of an accredited school of nursing, minimum bachelor’s degree in nursing.
- Current licensure as a Registered Nurse in the State of Vermont – unrestricted
- Evidence of one year of nursing experience, addiction medicine preferred
- BLS /CPR Certification
Key Responsibilities
- Facilitate the intake process for all new patients, including performing initial assessment, obtaining all necessary paperwork and labs, and scheduling initial appointment with the MOUD provider.
- Support patient engagement with treatment, healthcare and other supportive services.
- Conduct patient visits, as needed, with provider over site.
- Administers appropriate medication injections under the orders of the provider.
- Coordinate with insurance carriers to process prior authorizations for medication.
- Monitor the health, treatment progress and medication adherence of MOUD patients and participate in multidisciplinary team meetings.
