What You’ll Do:

• Direct Patient Care: Provide compassionate, hands-on care, from vital signs to patient education.
• Team Collaboration: Work closely with RNs, MAs, clinicians, and other healthcare team members to ensure seamless patient care.
• Patient Advocacy: Be a voice for our patients, supporting their needs and care plans.
• Documentation & Compliance: Maintain accurate patient records to ensure the highest quality standards.

What We’re Looking For:

• Education: Graduate of an accredited LPN program and an active LPN license in Vermont.
• Experience: Previous experience in community health, outpatient, or primary care is a plus.
• Skills: Strong communication, attention to detail, ability to multitask, and a commitment to compassionate care.
